登録原票記載事項証明書 Toroku Genpyo Kisai Jiko Shomeisho
外国人の人は、住民票にかわるものとして登録原票記載事項証明書があります。本人または本人と同居の家族で16歳以上の人が申請することができます。それ以外の人が申請する場合は、本人からの代理人選任届が必要です。申請は本庁舎市民課および上河内地域自治センター、河内地域自治センター、各地区市民センター・出張所で受け付けています。外国人登録証明書（本人以外の人の場合はその人の身分証明書）を持参してください。証明書の窓口での交付は一通300円、自動交付機での交付は一通200円です。
In lieu of the "jumin-hyo" resident registration slip, foreign residents have what is called a cer-
tificate of registered items in the Alien Registration (toroku genpyo kisai jiko shomeisho). This
certificate can be obtained upon application either in person or by a family member who is 16
years of age or older and lives in the same household. If someone else files an application, a
letter written by the applicant giving the third party the power of attorney is required. Appli-
cations are accepted at the Community Service Division of the City Office, Kamikawachi
Residents Association Center, Kawachi Residents Association Center, local residents
centers and branch offices. Bring your certificate of alien registration. (If someone other
than the applicant files the application, the representative must bring his/her own identifi-
cation card.) The handling fee for the certificate at a counter is 300 yen per copy. The hand-
ling fee for the certificate at an automated issuing machine is 200 yen per copy.
